Psoriatic Arthritis: A Complete Overview
Psoriatic arthritis affects up to 30% of people with psoriasis and can cause permanent joint damage if untreated. With more targeted biologics than ever before, early diagnosis and treatment have never been more important.
Psoriatic arthritis is an inflammatory autoimmune disease that attacks the joints, tendons, and ligaments usually in people who already have psoriasis. Left untreated, it can cause irreversible joint damage. With the right therapy, started early, most people can achieve remission or low disease activity.

What Is Psoriatic Arthritis?
Psoriatic arthritis (PsA) is a chronic autoimmune inflammatory arthritis closely linked to psoriasis. In 60 to 70% of cases, psoriasis precedes PsA by an average of 10 years. In about 15 to 20% of cases, arthritis actually appears before the skin disease. PsA is classified as a spondyloarthropathy and is distinctly different from rheumatoid arthritis in both biology and presentation.
Unlike rheumatoid arthritis, PsA affects men and women equally; is seronegative (no rheumatoid factor or anti-CCP antibodies); can involve the spine and sacroiliac joints; involves enthesitis and dactylitis; affects distal finger joints; and is associated with psoriasis and nail disease.
The Five Joint Patterns of PsA
PsA can present in five distinct joint patterns. A patient's pattern can change over time, and multiple patterns can coexist.
1. Asymmetric Oligoarthritis (most common at disease onset, ~70% of patients): Fewer than five joints affected, typically on different sides of the body.
2. Distal Interphalangeal (DIP) Predominant (~5 to 10% of patients): Inflammation primarily at the most distal finger and toe joints. Nail disease almost universally present.
3. Symmetric Polyarthritis (~15 to 25% of patients): Five or more joints affected symmetrically, closely resembles rheumatoid arthritis.
4. Spondylitis / Axial PsA (~5 to 35% of patients): Inflammation of the spine and sacroiliac joints, causing morning stiffness, back pain, and reduced spinal mobility.
5. Arthritis Mutilans (less than 5% of patients): The most severe form, characterized by osteolysis of the small joints of the hands and feet, leading to significant deformity.
The Six Domains of PsA
- Peripheral Joints: Swollen, tender, and stiff joints in hands, feet, knees, ankles
- Axial Disease: Spine and sacroiliac inflammation
- Enthesitis: Tendon/ligament insertion inflammation at the heel, knee, Achilles
- Dactylitis: Sausage digits, diffuse swelling of entire finger or toe
- Skin (Psoriasis): Plaques, scaling
- Nail Disease: Pitting, onycholysis, nail crumbling, affects ~80% with PsA
Enthesitis affects approximately 30% of PsA patients and is one of the most painful features, distinguishing PsA from RA.
Symptoms
- Joint pain, swelling, and warmth
- Morning stiffness lasting more than 30 minutes
- Dactylitis (entire digit swelling)
- Enthesitis (pain at tendon insertion sites)
- Lower back or sacroiliac joint pain
- Nail pitting, crumbling, or separation
- Psoriatic skin plaques
- Fatigue, often severe and debilitating
- Eye inflammation (uveitis) in 7 to 20% of patients
- Reduced range of motion and functional impairment
PsA can cause irreversible bone erosion and joint destruction within the first two years of disease onset. If you have psoriasis and develop joint symptoms, seek evaluation from a rheumatologist promptly.
Diagnosis
There is no single definitive test for PsA. Diagnosis is clinical, based on the combination of inflammatory arthritis, psoriasis (current, past, or family history), and the absence of other explanations. The CASPAR criteria are widely used. Supportive investigations include X-rays and MRI, musculoskeletal ultrasound, blood tests (ESR, CRP), and tests to rule out RA.
Treatment
The goal of treatment is remission or minimum disease activity (MDA) using a treat-to-target strategy.
Step 1 - NSAIDs and Physical Therapy: For mild disease, NSAIDs reduce pain and stiffness. Physical and occupational therapy preserve joint function.
Step 2 - Conventional Synthetic DMARDs: Methotrexate, sulfasalazine, and leflunomide slow disease progression for peripheral joint disease.
Step 3 - Biologic DMARDs: For moderate-to-severe PsA or when csDMARDs fail.
- TNF-alpha inhibitors (Adalimumab, Etanercept, Infliximab, Certolizumab): Longest track record; best for peripheral joints, axial disease, enthesitis, and skin.
- IL-17A inhibitors (Secukinumab, Ixekizumab, Bimekizumab): Excellent across all domains including axial. Avoid in Crohn's disease.
- IL-23 inhibitors (Guselkumab, Risankizumab, Tildrakizumab): Excellent skin clearance; IBD-safe.
- IL-12/23 inhibitor (Ustekinumab): Peripheral joints, skin; IBD-safe.
Step 4 - Targeted Synthetic DMARDs: Oral options for patients who prefer pills or have failed biologics. JAK inhibitors (tofacitinib, upadacitinib), PDE4 inhibitor apremilast, and TYK2 inhibitor deucravacitinib (strong Phase 3 results in 2025).
The Phase 3 POETYK trial showed deucravacitinib met its primary endpoint for PsA with 54% ACR20 response versus 34% for placebo. TYK2 inhibition is emerging as a promising oral option with regulatory review underway.
Comorbidities
- Cardiovascular Disease: Systemic inflammation accelerates atherosclerosis. Higher rates of MI, stroke, and hypertension.
- Metabolic Syndrome and Obesity: Reduces biologic response. Weight management improves outcomes.
- Depression and Anxiety: Affects up to 30% of PsA patients.
- Uveitis: Ocular inflammation affects 7 to 20% of patients. TNF inhibitors (excluding etanercept) are recommended.
- Inflammatory Bowel Disease: Co-occurs in ~10% of PsA patients. IL-17 inhibitors may worsen Crohn's.
- Osteoporosis: Bone density monitoring recommended, especially in axial disease.
Living Well With Psoriatic Arthritis
- Exercise regularly: Low-impact activity (swimming, cycling, walking, yoga) maintains joint function and reduces pain.
- Manage weight: Even modest weight loss of 5 to 10% improves drug effectiveness and reduces disease burden.
- Quit smoking: Smoking worsens psoriasis, increases PsA severity, and reduces biologic effectiveness.
- Build your care team: Optimal PsA management requires both a rheumatologist and a dermatologist coordinating treatment.
- Track and treat-to-target: Work with your rheumatologist on a clear treatment target and regular assessment.
Coordinated dermatology and rheumatology care is increasingly recognized as the standard of care for PsA. Both specialties should be engaged from the start.
